Summer Program Dates: Monday, June 26, to Thursday, August 3

To Register:

SELECT the class or classes you want from the list below. Only one parent/guardian need be entered to submit.
PLEASE provide cell phone info if you are not reachable at home during class time.

1. You will need to fill out one registration for EACH student being registered.
2. You will receive an automated receipt confirmation which indicates we have gotten your form data. This is NOT a placement confirmation.
3. Mail or deliver full tuition payment (check or cash) to be received within 7 days. Unpaid registrations may be cancelled if not paid within 7 days. If your registration has been cancelled you can register again at any time, subject to space availability, but payment will be due immediately at that time.

NOTE!
Phones and tablets work fine with this form, however many people have problems! The problem is most likely
AUTOCORRECT, which will change data in entry fields without you realizing it. It’s very common that people enter correct data in the security check box at the very bottom of the form only to have autocorrect alter it without them noticing. If your form will not submit, check the security field to be sure it’s accurate.
